Library Application Form

For Distance Education students only

Applying for a library card is easy--just follow the steps below:

  • Complete all sections of this application form.
  • Verify that all information is correct.
  • You can pick up your library card at one of our four libraries, or you can request that it be mailed to you.
Please Note:
You can pick up your Wake Technical Community College Library Card during normal hours or request that we send it to you. You will be required to present a photo identification, such as a driver's license, upon pickup. Before a card can be mailed to you, library staff will verify your enrollment as a distance education student in the span of one workday. All replacement cards have to be picked up. There is a $1.00 charge for card replacement.
